You will need to rely heavily on strategy. You get much less in the way of powers to work with, armor and weapon upgrades are a bit sparse as well, and expensive. Insanity in this game is by far the hardest of the three. The other important thing would be Paramour II (20G) for romancing someone, and ensure you've imported your character/continue playing on Insanity for those cumulative achievements. Outside of that, you can just run through the story and complete all the combat achievements. You'll also need to completely upgrade the Normandy and make the correct assignments during the suicide mission to ensure everyone survives. Be sure to talk with your squad after every main story mission as you will need to unlock and complete every loyalty mission. Your biggest worry here will be the No One Left Behind (75G) achievement for getting through the final mission with no allies dying. This makes things a bit less stressful and saves a whole bunch of time on the completion. The completion for this portion of the achievement list is mostly the same as the original releases, though some of the missable achievements from the "DLC" sections have been removed, and the achievements for the Arrival content were removed completely with that being integrated directly into the story. The only difficult enemies can be the large Geth or Krogan enemies that charge you. Most enemies just become bullet sponges with little to no strategy behind their movements. You generally get equipment upgrades often, your allies have a lot of powers to use, and your weapons can dish out huge damage. Insanity difficulty for this game is by far the easiest of the three. This may not be an issue on the most patched version of the game, but see those achievements for specifics on an easy grinding method if you'd just like to get it out of the way. There seems to be some trouble with these keeping tracking when moving between games, and both can be done in less than an hour anyway.
